Locations:

Working hours:
- Full- Time, 39 Hours per week
Contract:
- Permanent
The successful candidate will work closely with the Social Care Specialist in delivering of activities to the resident and provide support and training to the Care team when needed.
Duties Include (but are not limited to):
Social
- Develop a schedule of exciting events and activities for residents at Four Ferns.
- Ensuring the activity calendar is planned in advance
- Providing a broad range of activities to cater for all needs including both group and individual activities.
- To take a collaborative approach to activity planning, listening to residents, staff and family.
- Direct provision of activities to all residents within the home.
- Report any changes in resident’s condition to the trained nurse.
- Escort or transfer residents to aid their participation in activities employ manual handling best practice and take direction from the Nursing staff
- Lead and develop a social programme for all the residents living in Four Ferns.
- Ensure the scheduling of social sessions provides maximum benefit to both the resident and the care team
- Assist residents in socialising within the home and community
- Encourage staff members, relatives, and friends to participate in the home’s activities with Residents.
- To plan ongoing weekly activity rotas in conjunction with the Residents wishes and encourage Residents to maintain pre-existing hobbies.
- Record and update resident’s documentation on engagement levels during activities.
- Support and assist the management team during internal and external audits/inspections.
- Be committed, patient and caring person with a sense of humour and a positive outlook on life
- Assess residents needs continuously, particularly with regard to possible rehabilitation and participate in resident reviews as appropriate.
- Foster good community relation and manage the volunteer programme in line with the centres policy
- Encourage resident to join in meaningful activities whilst maintaining their independence, choice and dignity, to enable them to lead as full and active life as possible
- Consult with each resident in order to identify their social and recreational requirements according to their likes/dislikes, abilities and personal preferences.
- Create monthly programme of activities and social events and ensure it is communicated to all residents.
- Regular assessment of the effectiveness of the social events and activities programme.
- To work as a key member of the multidisciplinary team in order to feedback on each resident’s status/participation at regular care plan reviews
Administration
- To familiarise and connect with all key community groups whose expertise would make a valuable contribution to the service.
- To manage and schedule care staff within the centre on day to day supervision
- To support volunteers in their role and ensure they receive appropriate supervision and support.
Requirement
- Ability to develop and organise an enjoyable activities schedule for residents.
- Qualification or relevant experience in Social Care
- Previous experience working with the older person an advantage
- Highly efficient and organised
- Excellent interpersonal skills
- Experience in Dementia Care
- Ability to plan and lead in activities /social planning in line with Resident’s choice and ability.
Desirable:
- Dementia Specific training
- Degree / masters level programme related to Social / Healthcare
